Principal Responsibilities:

The Weinberg Center for Balanced Living (WCBL) is an innovative Older Adult Center serving adults ages 60 and older, whose primary languages include English, Cantonese, Mandarin, Spanish, and Russian. Integrated into EA's Manny Cantor Center (MCC) and funded by the New York City Department for the Aging (NYC Aging), WCBL offers a holistic program designed to meet the varied needs, abilities, and interests of older adults. WCBL's programs make it a premier destination for older New Yorkers, supporting physical and nutritional health, artistic development, cultural and social engagement, and providing critical social services that help members age in place with dignity.

We are seeking an experienced Senior Social Worker Supervisor, LMSW, who will play a key role on an interdisciplinary team supporting the holistic wellness of older adults. The Senior Social Worker Supervisor will maintain a caseload of older adult clients from WCBL and EA 202 Housing (Podell and Alliance Apartments). Assist clients with a range of complex issues related to aging: facilitate client-led goal setting and attainment; provide concrete case assistance, referrals, and routine follow-up; and provide psychoeducation, advocacy, and supportive contact. Coordinate and/or develop and facilitate regular group workshops for clients to address shared needs and promote community well-being.

RESPONSIBILITIES

  • Provide onsite task supervision to the WCBL social services team.
  • Participate in case conferences and/or group supervision.
  • Appropriately assign cases to WCBL social services staff and interns.
  • Maintain a caseload of older adult clients: conduct intakes and comprehensive assessments of clients' strengths, needs, and challenges; formulate ongoing care plans and recommend appropriate intervention strategies; and routinely follow-up.
  • Provide case assistance, assisting clients to obtain concrete services, such as completion of applications/renewals for housing and benefits and entitlements; and complete related forms in the NYC Aging electronic record.
  • Make strategic referrals and advocate for service type, frequency, and intensity.
  • Provide supportive contact to help clients address challenges with daily living.
  • Facilitate or co-facilitate relevant psychosocial, psychoeducational, and/or mental health workshops and/or group series for community members.
  • Maintain documentation following funder's prescribed format and time guidelines: update client information and enter supervisory case review notes, client case notes, and units in electronic record; and complete case review, case assistance, and information and referral logs.
  • Work collaboratively as part of the Older Adult Programs team, providing office and meal program support when needed, participating in the organizing and executing of WCBL events, and coordinating workshops and events at EA's 202 Housing.
  • Coordinate with internal partners and external agencies to ensure best care for clients and provide professional development opportunities for staff.
  • Attend supervisory, program, agency, and community meetings as required.
  • Provide or ensure access to interpretation and translation services for clients.
  • Perform other related duties as assigned by supervisor(s) and/or WCBL director.
