Principal Responsibilities:

Overall Description:

The Social Worker delivers Case Assistance and Case Management services to all older adults age 60 and over living in the NORC development and assists with programs and group activities as assigned. The Social Worker is instrumental in furthering the NORC program's mission to enhance the quality of life and independence for older adults as they age in place in their community. The Social Worker works under the supervision of the Program Director.

Location: Senior Services at Amalgamated/ Park Reservoir Houses - NORC Program Under the Sponsorship of the Riverdale Y

35 Hours per week

Salary range: $50,000 - $62,000

Reports to: AmPark NORC Program Director

Minimum Qualifications:

Duties and Responsibilities:

Engage older adults in the NORC office, and in other community settings to
gather information about their strengths, needs, abilities, and preferences
Assess older adults' presenting problems using a multidimensional psychosocial
assessment
Partner with older adults to establish a plan of care to address presenting
problems
Implement interventions indicated by the plan of care, including benefits and
entitlements, advocacy, referral to community resources, problem-solving, and
supportive contacts
Maintain regular contact with older adults to ensure timely follow-up to presenting problems
Update older adults' contact information, consents, and assessment data on an annual basis
Evaluate older adults' plan of care to ensure that the services provided are effective
Participate in case conferences and weekly team meetings with NORC program staff
Provide regular updates to the Program Director about older adults' presenting problems
Respond to crisis situations as appropriate
Assist and/or facilitate group programming
Document all services provided to and on behalf of seniors in the electronic database via detailed case notes and worker logs
Maintain confidentiality of all client information and adhere to New York City
Department for the Aging policies and professional standards of conduct
Attend NORC Partners Meetings and trainings with outside agencies as needed
Other duties as assigned

Qualifications:

MSW required. LCSW preferred
Knowledge of older adult issues strongly encouraged
Bilingual English/Spanish or English/Russian preferred
Strong interpersonal and organizational skills
Excellent written and verbal communication skills

Salary:

$55,000 - $65,000

The Riverdale Y is a 75,000 sq. ft. Jewish Community Center that provides educational, recreational, cultural, and wellness programs for children and adults of all ages and backgrounds. The Y features an early childhood center; summer camps; large after school program; senior center; performing arts center featuring theater, dance and music schools; and a health & wellness center that includes cardio and free weight studios, group exercise classes, gymnasium, swimming pool, steam room, sauna and massage therapy.
